BRHA COVID-19 RTP Acceptance

This form is for the formal notification that your club is adopting the BRHA's COVID-19 return to play policy, including the uploading of your respective risk assessment and corrective action plan. Only once this has been completed in full and you have received the approval email will your club/team and its members be eligible to return and insured.

The form will allow you to enter multiple team names in multiple age groups but, ultimately, you only need to submit one risk assessment and corrective action plan per club if all teams train and play in the same venue as the other. Any difference in risk or hazard between teams/age groups must still be clearly documented.

Ahead of your return, you must: -
1) Download a copy of our return to play policy;
2) Complete a risk assessment & corrective action plan;
3) Complete this acceptance form and upload all supporting documentation;
4) Arrange for all players and team officials to register on https://brha.co.uk/register;

Only once all steps are complete and you receive BRHA RTP approval by email will you be eligible to return and insured for liability and sports injury.

A copy of this response will be sent to the provided email address upon submission.
